Incidence and mortality rates for prostate cancer declined again in ...

The latest actual (as opposed to projected) data on the incidence and mortality rates for prostate cancer specifically (and cancers as a whole) continue to show?steady improvements here in the US.

According to the annual analysis by Eheman et al. just published in Cancer, and based on actual data from 2008, there were continued decreases in the incidence and the mortality rates for prostate cancer during that year compared to earlier years. These decreases are observed among black and white men as well as for the male population as a whole. The actual age adjusted incidence of prostate cancer for 2008 (relative to the year 2000) was reported as 152.9 per 100,000 men overall. However, for the black male population it continues to be significantly higher at 230.8 per 100,000.

Unless you are an epidemiologist with significant training in statistics, this is a difficult paper to follow since most of the data are based on a process called joinpoint analysis (which is far beyond the capabilities of your current author to explain). It also makes these data difficulty to compare to the data originally projected for 2008 by the American Cancer Society.

While it is good to know that the incidence and mortality rates for prostate cancer are continuing to fall slowly, it is also fair to say that progress is less than dramatic, and dramatic progress is going to need diagnostic tests that are a great deal more specific and accurate than the PSA test and needle biopsies and treatments that are able to profoundly impact disease progression of the more aggressive forms of prostate cancer. In real terms, the way we treat prostate cancer today (by comparison with the ways we now treat complex cardiovascular conditions) is still rather like using a sledge hammer to open a walnut.

Industry report warned of North Sea gas leak risk

The Elgin drilling platform in the North Sea has been leaking natural gas since 25 March, and is still at risk of catching fire or even exploding.

Total, the company in charge of the platform, had already experienced problems at the site. A report written by one of its drilling managers and published last year describes how difficult it is to pump gas from the deep Elgin field. It even explains exactly how drilling operations can damage the wells in ways that may have led to this week's leak.

Demand for natural gas has risen steadily over the last 30 years, and global gas production has more than doubled in that time. Gas producers like Total have turned to increasingly difficult reserves to meet those demands.

The Elgin field is one of those difficult reserves. At 5500 metres it is among the deepest active gas fields in the North Sea. Temperatures there reach 200 ?C and the gas is under 1100 bars of pressure. Last year's report explains that working with this kind of high-pressure/high-temperature (HP/HT) field is a particular challenge (Journal of Petroleum Technology, vol 63, p 116).

Once gas extraction begins, the pressure within any gas field will naturally begin to fall, but the drop is particularly acute in HP/HT reservoirs. This means that pressure at depth can vary dramatically and unpredictably once extraction begins, which makes it difficult to sink additional wells at a later date without damaging the expensive drilling equipment. To extract as much gas as possible, companies normally drill several wells into the reservoir before opening any of them. This ensures that they have backup wells, should some fail.

Wells failed

In fact, many of the wells in the Elgin field and the neighbouring Franklin field have failed since Total began extracting gas there in 2001, according to the report. The casings of some of the wells became badly deformed and their internal diameters shrank by 60 per cent, like damaged drinking straws. Others may have sheared off completely.

That damage was probably a direct result of gas extraction activities. The drop in pressure as gas was removed deformed the chalk layers above. Because the wells must pass through the chalk to reach the gas fields below, they too become deformed.

The damage to the well casings may explain this week's leak. We now know that its source is not the Elgin gas field itself, but rather a pocket of methane gas trapped within the chalk layers above. This methane somehow entered the casing of one well and travelled up to the Elgin platform.

The leaky well in question was actually closed over a year ago, and sealed at depth. But because Total continued to extract gas from the Elgin field, shearing forces within the chalk above may have cracked open the abandoned well-casing 4000 metres down ? 1500 metres above the main gas field ? allowing methane within the chalk to seep into the well and escape to the surface.

"Based upon the information we have available, I think it is highly probable this has happened," says Jon Gluyas of Durham University in the UK, who has 28 years of experience in the petroleum industry.

At the time of publication, Total had not responded to a request to comment on the relevance of last year's report to the current leak.

"Because of the demand for oil and gas, we will see this type of field exploited more often," Gluyas says. However, that does not necessarily mean we will see many more accidents along the lines of Elgin. Rather, the gas industry may improve its practices, for instance monitoring compaction in chalk layers more closely. "The industry will learn from this substantially," Gluyas says.

PARIS (Reuters) - A major exhibition on Henri Matisse opening in Paris this week brings a new perspective to his work, showing the painstaking creative process behind the simple shapes and strident colors that made the French artist's name.

"Matisse: Pairs and Series" explores for the first time how one of the greatest painters of the 20th century would repeat the same composition again and again, varying color and technique, before being satisfied with the result.

For a man best known as leader of the Fauvist movement, and for seemingly spontaneous bursts of color such as his 1906 "Joy of Life," the show reveals an insecure and restrained side that would remain unchanged throughout a six-decade career.

"We wanted to challenge the received wisdom that he was a happy painter, an easy painter, a sort of virtuoso of simplicity and joy," said exhibition curator Cecile Debray.

"Matisse was obsessive, worried, racked with doubt from the beginning to the end, and he showed it in his painting through this constant multiplication and exploration," she said.

The exhibition, expected to be one of the highlights of the spring cultural season in Paris, runs to June 18 at the Pompidou Center, before moving on to Copenhagen's Statens Museum for Kunst and then the Metropolitan Museum of Art in New York.

It comprises some 60 paintings and 30 drawings, including emblematic works like Matisse's "Blue Nude" cut-paper series from 1952, four collages labeled I to IV, where he began the last work first then tried three different variations, before coming back to finish his first attempt at the end.

"A painting is like a card game: you should know from the beginning what you wish to achieve at the end. Everything should be worked backwards so that you have finished before you have begun," Matisse is quoted as saying in the exhibition.

"WILD BEAST"

Although the idea of the tortured artist is perhaps nothing new, what's particularly surprising in Matisse's case was his willingness to show the laborious process behind his art.

In 1945, as the exhibition shows, Matisse displayed six of his finished works in a gallery in Paris, alongside framed photographs of their previous stages, showing the genesis of the work and the complexity behind the apparent ease.

Debray attributes the rare honesty to the artist's need for validation after the 1905 Autumn Salon in Paris, where the violent colors of works such as his "Open Window" and "Woman with a Hat" earned him and fellow artists the derogatory nickname "fauves" or "wild beasts."

"He was considered a bit of a joker and there was a real scandal. Maybe (fellow artist Marcel) Duchamp would have loved it, but Matisse wasn't like that and he kept trying to explain himself, to be taken seriously," Debray told Reuters.

Despite the early criticism, Matisse went on to achieve fame and fortune during his lifetime and is often regarded as the most important French painter of the 20th century.

Born in 1869 in Picardy in northern France, he studied law before discovering a love of art in 1890, while recovering from a bout of appendicitis, later achieving success with his first exhibition in 1896 with his "Woman Reading"

After coming under the influence of the Neo-Impressionists or Pointillists, he went on to research new techniques, eventually developing the broader brush strokes and passion for pure color that would characterize his work.

Matisse died in 1954 in Nice on the French Riviera.
